The short answer
Build this plan with the person living with dementia, not around or instead of them. Record their own preferences, the exact operating route, every companion or staff handoff and an accepted response to disruption or separation. A diagnosis does not determine travel fitness, legal authority, supervision or one universal assistance need. Resolve clinical, medicine and capacity questions through the responsible professional or authority. Keep the essential journey usable without a phone or network. Build the real route with the traveller
Start with the traveller’s own communication, routine, rest, privacy and support preferences. Write the actual sequence: home departure, operating carrier, terminal or station, security, border control, seat, connection, baggage, ground transfer, accommodation and return. Name who is expected at each transition and what the traveller has agreed may be shared. Do not infer comprehension, consent, legal capacity or fitness to travel from a dementia diagnosis. Confirm assistance and every handoff
Contact each actual airline, airport, railway, coach, ferry, transfer and accommodation through an independently verified channel. Prepare a protected offline journey packet
With the traveller’s agreement, prepare the minimum information needed for the journey: preferred form of address, useful communication prompts, itinerary, booking references, assistance confirmations, accommodation address, named companions, meeting points and independently verified contacts. Keep identity, health and account details private; a visible card should not become a complete medical file. Make change and separation plans work without connectivity
Choose permitted, staffed meeting points for every terminal, station, attraction and accommodation handoff. Agree what the traveller and companion will do if a gate, platform, carrier, hotel, country or arrival time changes. Reconfirm the new route with the responsible provider instead of improvising an unverified pickup or asking a stranger to take control. If separation actually occurs, move immediately to the site’s competent staff, police or local emergency process; the existing missing-companion workflow owns that live incident. Measure only the ordinary data left after the safe plan
After the route, assistance, personal support, offline packet and separation response all work without connectivity, list only non-critical online tasks: checking an authenticated timetable, refreshing an ordinary map or messaging accommodation.
